Create a form

In this article, we explain how to create and publish a new form.

You can create new forms such as care plans, incident forms and risk assessments.

To get started, follow these simple steps:

  1. Click three-line menu.

  2. Click Configuration, then click Forms.

  3. Click New Form or select the form and click Edit Form.

    πŸ€“Tip: When filling in the form settings, if you select the Required Form checkbox, you can't end a visit without completing the form.

  4. Enter a form name and header, then on the left, click Fields.

  5. Drag and drop the fields into the empty form on the right.

  6. Click the field you need to amend then click Field Settings.

  7. Enter the name and any instructions for the field.

    πŸ€“Tip: The name is under label text.

  8. To make the field required, check the required checkbox.

  9. Click Save to save a draft or click Publish to use the form.

πŸ“ŒNote: When editing a form this doesn't affect historical form submissions only if submitted against a service user or employee form is edited.

Component type

Description

Text Box

Free text field with a character limit of 1,000 characters.

For example: Name, Address, NHS Number.

Number

Only accepts number values.

Doesn't accept decimal numbers.

Paragraph Box

Free text field with no character limit.

For example: Allergies, Details of medication, and condition.

Decimal

Accepts decimal values only.

Multi-Select

Allows the user to select more than one value from the list of options available.

Dropdown

Allows user to select only one value from the list of available options.

Time

Allows user to input time value only.

Date

Accepts date values only.

Radio Button

Allows user to select only one value from the list of available options.

Checkbox

Allows the user to select more than one value from the list of options.

Signature

Allows user to add a signature to the form - App only.

Photo

Allows users to submit an image when completing the form.

Barcode

Allows the user to scan a barcode and add the barcode number to the form - App only.

Hyperlink

Add a link to a website within the form.

Label

Allows users to add informational data to the form.

For example: Consent Agreements.

Client Picker

This feature is currently not available on app. If you're making forms for use in the app, we advise you to refrain from using this component.

Employee Picker

This feature is currently not available on app. If you're making forms for use in the app, we advise you to refrain from using this component.

Table

Add a table with multiple columns to the form. Rows can only be added when completing the form.

Section

Allows the user to add structure to a form by adding components to sections.
