Handover setup

In this article, we explain how to set up and complete a handover.

This module simplifies shift handovers by using care plans and ad-hoc activities within Access Care Planning, saving time and improving consistency. Each branch has one centralised handover setup, accessible only via the web console. Staff involved in handovers, such as team leaders, nurses, or seniors, must have web console access and be assigned as a branch administrator or a tenant administrator to view and complete them.

Setup

To set up a handover, follow these simple steps:

  1. Click on the three-line menu.

  2. Click Configuration, then click Branches.

  3. Select the relevant branch.

  4. Click Add Shift, then create a code and description and add a start time.


Configure shift activities

Once the shift patterns have been added, you'll need to choose which activities are automatically included. Any of the activities you have defined to show on the handover automatically pull through to the correct handover shift based on the time completed, whether they are scheduled or ad-hoc.

To add activities, follow these steps:

  1. Click the three-line menu.

  2. Click Configuration, then click Tenant.

  3. Click Standard Activities.

  4. Select or create an activity.

  5. Check the In Handover? Checkbox.


Add records

Now that the Handover feature has been configured, it automatically creates a handover record at the start of the next shift. At this point, you need to choose which service users to include in the handover. To do this, follow these steps:

  1. Click on the three-line menu.

  2. Click Handover, then choose the relevant branch.

  3. Select the handover you've just created.

    πŸ€“Tip: Handovers won't be created until the shift time occurs.

  4. Click Add Residents.

  5. Search for the relevant service users.


Configure records

Once service users have been added to the handover, you can add in important information for each service user which shows on all future handovers. This can be edited at any time to ensure the most important information is showing.


Use Handover

Now that you have configured a handover, the activities you chose to include are automatically reflected on the handover when completed against a particular client throughout a shift. You'll also see information regarding the previous 24 hours’ worth of shifts. Any scheduled or Ad Hoc completion of the defined activities is reflected in the handover for the shift.

You can click into an activity to see further information and any comments left against it.

To the right of the screen, you can leave notes against a service user for the shift, you can also flag a shift or mark it as complete.
