Smart Notes Form Optimisation Guide

Smart Notes works better when you tell it what you want. Every form field in ACP has an "Instructions" box where you can guide the AI.

These instructions help Smart Notes understand exactly what information belongs in each field.

In your ACP form builder, every field has an Instructions tab:

Simply add clear guidance in this box. Care workers will see these instructions on their forms, and Smart Notes uses them to place information correctly.

Before: "Does the client need help with meals?"
​After: "Does the client need help with meals? Say yes or no. If yes, describe the help needed."

Before: "Morning routine"
​After: "Morning routine - State wake time, then personal care needs, breakfast routine, and medications. Keep to 3-4 sentences."

Before: "Current medications"
​After: "Current medications - List each medication with a pause between each one."

Before: "Next review date"
​After: "Next review date - Say the full date like '15th January 2025'"

Without instructions: 60-70% accuracy, 10-15 minutes fixing errors
​With instructions: 80-95%% accuracy, 2-5 minutes, minor tweaks

"Information appears in the wrong fields"
Add clearer instructions to each field: "Only include [specific information] here"

"Too much or too little detail"
