What You Need to Know

Smart Notes works better when you tell it what you want. Every form field in ACP has an "Instructions" box where you can guide the AI.

These instructions help Smart Notes understand exactly what information belongs in each field.

How to Add Instructions

In your ACP form builder, every field has an Instructions tab:

Simply add clear guidance in this box. Care workers will see these instructions on their forms, and Smart Notes uses them to place information correctly.

Quick Wins - Update These First

Yes/No Questions

Before: "Does the client need help with meals?"
After: "Does the client need help with meals? Say yes or no. If yes, describe the help needed."

Open Text Fields

Before: "Morning routine"
After: "Morning routine - State wake time, then personal care needs, breakfast routine, and medications. Keep to 3-4 sentences."

Lists and Multiple Items

Before: "Current medications"
After: "Current medications - List each medication with a pause between each one."

Time and Date Fields

Before: "Next review date"
After: "Next review date - Say the full date like '15th January 2025'"

Which Forms to Update First

  1. Daily visit notes - Used most frequently

  2. Initial assessments - Save the most time (60+ minutes each)

  3. Care plan reviews - Monthly requirement

  4. Risk assessments - Critical for compliance

Common Problems - Quick Fixes

"Information appears in the wrong fields"
Add clearer instructions to each field: "Only include [specific information] here"
