Understanding the GP Connect record in Access Care Planning

See what data is available in each section of a service user's GP Connect record.

Once you've linked a service user to GP Connect, their NHS GP record is available directly within Access Care Planning. The record is read-only and is sourced in real time from NHS systems. You won't be able to edit any of the information shown here.

The record is split into tabs across the top of the screen. This article explains what each tab contains.

Please note: All data shown in GP Connect is sourced directly from NHS systems. Access Care Planning does not modify or store this information. If you believe any data is incorrect, please raise it directly with the service user's GP practice.

Summary

The Summary tab gives you a quick overview of the most clinically relevant information. It includes:

  • Emergency Codes - any nationally defined codes flagged against the record

  • Last 3 Encounters - the three most recent GP appointments or interactions

  • Active Problems and Issues - current conditions being managed

  • Major Inactive Problems and Issues - significant conditions that are no longer active

  • Current Allergies and Adverse Reactions - known allergies recorded at the GP practice

  • Acute Medication (Last 12 Months) - short-course medications prescribed in the last year

  • Current Repeat Medication - ongoing medications currently prescribed

This is a good starting point before reviewing any individual tab in more detail.

Encounters

This tab lists all recorded GP encounters for the service user. Each entry includes the date, the clinician involved, and a summary of the discussion or actions taken.

You can filter by date range using the From and To fields, then click Filter to narrow the results.

Clinical Items

Clinical Items show diagnostic tests and investigations recorded by the GP practice. Each entry includes the date, the test or investigation type, and the recorded result.

Use the date filter to view results within a specific period.

Problems and Issues

This tab breaks conditions down into three groups:

  • Active Problems and Issues - conditions currently being managed

  • Major Inactive Problems and Issues - significant conditions that have ended or resolved

  • Other Inactive Problems and Issues - minor or resolved conditions

Each entry shows the start date, significance, and treatment notes where available.

Allergies and Adverse Reactions

This tab shows all allergies and adverse reactions recorded at the GP practice, split into:

  • Current Allergies and Adverse Reactions - active allergies

  • Historical Allergies and Adverse Reactions - past reactions that are no longer flagged as current

This information is sourced directly from the GP record and is separate from any allergy information recorded within ACP itself.

Medications

The Medications tab provides a full picture of a service user's prescription history. It is split into four sections:

  • Acute Medication (Last 12 Months) - short-course medications prescribed recently

  • Current Repeat Medication - ongoing prescriptions currently active

  • Discontinued Repeat Medication - repeat prescriptions that have been stopped

  • All Medication - a complete list of all medications, both active and historical

  • All Medication Issues - individual prescription issue records, including quantities and durations

Use the date filter to review medication history within a specific period.

Referrals

This tab shows referrals made by the GP practice on behalf of the service user. Each entry includes the date, the referring practice, the destination service, priority level, and any additional details.

Use the date filter to narrow down referrals within a specific time period.

Observations

Observations show recorded measurements and assessments from the GP record, such as questionnaire responses or health checks.

Each entry includes the date, observation type, recorded value, and any relevant details.

Use the date filter to view observations within a specific period.

Immunisations

This tab lists all vaccinations recorded at the GP practice.

Each entry shows the date, the type of vaccination, and any available details about contents or administration.

Administrative Items

Administrative Items shows non-clinical entries from the GP record, such as reminders or administrative flags. These are typically used by the GP practice for internal tracking purposes.

Use the date filter to view items within a specific period.
