Evo Assistant Builder Templates for Access Care Planning

An overview of the three Evo Assistant Builder templates available for Access Care Planning during our early adopter programme - covers what each AI co-worker does, who it is for, and how to get started.

During the Early Adopter Programme (EAP), three ready-made Assistant Builder templates are available for Access Care Planning. Each template is a pre-configured AI co-worker focused on a specific care management task.

Your Access representative will set up and configure the assistant during onboarding. Once it is ready, you access it via the Agent Builder test chat and ask questions in plain English.

Care Planning and Documentation Agent

An AI co-worker that helps care workers create accurate, person-centred visit notes. Describe a visit in plain English and the agent queries your live ACP data, prompts for any missing detail, and suggests a structured visit note for you to review and submit.

What it does:

  • Describe a visit in plain English - what happened, who was seen, what was done.

  • The agent queries ACP for the service user's care plan, visit history, and outstanding activities to give context to your description.

  • If key detail is missing - such as the outcome of an activity, a concern, or the status of a medication - the agent prompts you to add it before producing a draft.

  • Returns a structured visit note in plain English, ready for you to review and submit via ACP or Navigator Quick Form Submit.

  • Flags any medication activities that were not recorded during the visit so they can be followed up.

Who it's for: Care workers and senior carers responsible for recording visit notes, particularly those who find documentation time-consuming or who are covering for an absent colleague.

Value: Care workers spend less time writing notes and more time on care. Record quality improves because the agent prompts for missing detail before a draft is produced. Workers who find documentation difficult get a structured scaffold rather than a blank form.


CQC Assurance and Compliance Agent

An AI co-worker that queries your ACP compliance data on demand, surfaces gaps and patterns, and helps you build inspection evidence - without manual report-pulling.

What it does:

  • Ask a compliance question in plain English - for example, "Which service users are missing a current risk assessment?" or "Show me care plan review completion rates for the last 90 days."

  • The agent queries ACP for care plan review dates, risk assessment status, activity completion rates, incident records, and key dates.

  • Returns a structured response covering completion rates, gaps, flagged risks, and overdue items - organised by CQC domain where relevant.

  • Explicitly flags where data is absent or incomplete so you know what is missing, not just what is present.

  • Every response includes a disclaimer that findings should be verified against source records before use in formal reporting.

Who it's for: Registered Managers and Registered Nurses responsible for CQC compliance, inspection preparation, and ongoing quality assurance.

Value: Compliance status that previously required manual report-pulling and cross-referencing is available in seconds. CQC preparation becomes a continuous process rather than a pre-inspection scramble. Inspection-ready evidence is always current.


Last Visit Summary Agent

An AI co-worker that gives care workers a concise briefing on a service user's last visit before they arrive - covering what was done, any concerns, outstanding activities, and recent care plan changes.

What it does:

  • Ask for a briefing on a named service user - for example, "What happened on John's last visit?"

  • The agent queries ACP for the most recent completed visit record, outstanding activities, current care plan highlights, and any alerts or incidents since the last visit.

  • Returns a concise bullet-point briefing covering: last visit date and attending carer, summary of what happened, activities completed and missed, concerns or flags raised, care plan changes since the last visit, and outstanding actions.

  • Explicitly notes if the last visit was missed or amended rather than completed as planned.

  • Surfaces active alerts and recent incidents prominently so nothing critical is missed before arrival.

Who it's for: Care workers and senior carers, particularly those covering for an absent colleague or returning to a service user they have not visited recently.

Value: Care workers arrive with full context rather than relying on memory or printed handover notes. Continuity of care improves. Workers covering for absent colleagues have immediate visibility of what happened last time and what needs to happen next.


How it works

All three templates follow the same pattern:

  1. Your Access representative creates and configures the assistant during EAP onboarding.

  2. Access the assistant via the Agent Builder test chat.

  3. Ask a question or describe a task in plain English.

  4. The assistant queries your live ACP data and returns a structured response.

Important: The assistants query your live ACP data. The Care Planning and Documentation Agent can both read and write to ACP. All other assistants are read-only during EAP. Any notes or records produced by an assistant must be reviewed by a qualified member of staff before being committed to the care record.


Known limitations

The following limitations apply during the Early Adopter Programme (EAP).

  • Tenant administrators only. Assistant Builder is currently available to tenant administrators as part of the EAP. Access will expand during the general availability release.

  • Agent Builder test chat only. Assistants are accessed via the Agent Builder test chat during EAP. Navigator Chat integration is planned for a future release.

  • Desktop web only. Evo Assistant Builder is available on desktop and laptop browsers only during EAP.

  • Navigator Chat behaviour. If a service user asks a question via Navigator Chat that overlaps with an assistant's scope, Navigator Chat may respond differently from the configured assistant instructions. This is expected behaviour during EAP.
