During the Early Adopter Programme (EAP), three ready-made Researcher project templates are available for Access Care Planning. Each template is pre-configured for a specific care management task. Your Access representative will set up the project during onboarding - once it is ready, you upload your documents and start asking questions.

Care Plan Workspace
A single Researcher project that covers four common care plan document tasks. Export documents from ACP, upload them into the Care Plan Workspace, and ask questions - the AI has full context of everything you have uploaded.
The four tasks covered by this template:
Drafting a person-centred care plan from uploaded assessment notes
Reviewing a care plan for quality against CQC person-centred standards
Creating a service user summary from recent notes and a care plan
Drafting Local Authority review pack content from uploaded outcomes evidence and correspondence
What it does:
You export a document from ACP - a care plan PDF, assessment notes, risk assessment, outcomes evidence, or LA correspondence -and upload it into the project.
Documents are processed automatically on upload. You can then ask questions in plain English.
For a quality review, Researcher identifies missing sections, generic language, and gaps against person-centred standards.
For a draft care plan, Researcher returns a structured draft in plain English appropriate for ACP care records.
For a service user summary, Researcher returns a concise overview covering care needs, preferences, risks, and outstanding actions.
For an LA review pack, Researcher returns structured draft content for the relevant review sections.
Who it's for: Registered Managers and Care Coordinators responsible for care plan quality, review compliance, and Local Authority reporting.
Value: Four previously separate document tasks are available in one project. A CQC-standard quality review that previously required manual cross-referencing takes under a minute. Drafting a care plan from uploaded assessment notes - a task that can take hours - is reduced to minutes.
Medication Error Investigation
A Researcher project for Registered Managers investigating a medication error or near-miss. Upload the relevant evidence documents and ask for a structured chronology, root cause analysis, and corrective action suggestions.
What it does:
You export relevant records from ACP - eMAR records, incident reports, submitted forms - and upload them alongside any external documents such as staff statements, policy extracts, and MAR charts.
Ask for a chronology of events and Researcher returns a timeline drawn from the uploaded documents in chronological order.
Ask for root causes or contributing factors and Researcher returns a structured list without assigning personal blame.
Where evidence includes eMAR records, Researcher can cross-reference what was prescribed against what was administered.
If the uploaded content describes a serious medication error, Researcher flags any potential safeguarding or regulatory reporting obligations.
Who it's for: Registered Managers investigating medication incidents in domiciliary or residential care settings.
Value: Medication incident investigations that previously required hours of manual document assembly produce faster, more consistent output. Managers produce structured, defensible investigation records rather than ad-hoc written summaries - directly supporting CQC evidence requirements.
Creating Accessible Communication
A Researcher project for rewriting complex care documents, policies, or information into plain, accessible language for staff, clients, or family members.
What it does:
You upload a policy document, care plan, or information notice.
Ask for a plain-language rewrite for a specific audience - for example, a client with a learning disability, a family member, or a new care worker whose first language is not English.
Researcher returns a rewritten version using simple vocabulary, short sentences, and active voice, while retaining all factual accuracy from the original document.
Who it's for: Registered Managers, Care Coordinators, and anyone responsible for producing accessible communications for clients, families, or staff.
Value: Accessible versions of care plans, policies, and communications can be produced without specialist writing support. Supports inclusivity for clients with learning disabilities, dementia, or low literacy, and for staff where English is not a first language.
How it works
All three templates follow the same pattern:
Your Access representative creates and configures the Researcher project during EAP onboarding.
You export the relevant documents from ACP and upload them into the project.
Documents are processed automatically. You can start asking questions straight away.
Researcher returns a structured response based on the content you have uploaded.
Important: Researcher works from the documents you upload. It does not connect directly to your live ACP data. If records in ACP are updated after you upload, you will need to export and upload again to reflect those changes.
Known limitations
The following limitations apply during the Early Adopter Programme (EAP).
Tenant administrators only. Researcher project setup is currently managed by your Access representative during EAP. Self-serve project creation will be available at general availability.
Web Browser only. Evo Researcher is available on desktop and laptop browsers only during EAP. Mobile access is not supported at this stage.
