Access Evo Navigator is made up of a set of apps, each focused on a specific part of your daily work.
During the Early Adopter Programme (EAP), you have access to the 10 Access Care Planning focused, Evo Navigator apps described below.
Note: Navigator apps are available on desktop and laptop browsers only during EAP. Mobile access is not supported at this stage.

Form Updates
See a live feed of form submissions across your service users - filtered by staff member or service user, with the ability to preview any form inline and jump straight to the relevant case in ACP.
What it does: Displays all recent form submissions in one place, so you do not need to open individual records to check what has been submitted.
Who it's for: Registered Managers and Care Coordinators who need to monitor documentation activity across their caseload.
Value: Reduces the time spent checking whether forms have been completed - everything surfaces in one feed.
Short video walkthrough:
Smart Form Builder
Convert paper or document-based care forms into ACP-compatible digital forms - without using Forms Designer. Upload a Word, Excel, PDF, or image file and get back a ready-to-import JSON file for ACP.
What it does: Uses AI to extract the structure and fields from an uploaded document and exports them in the exact format ACP needs. No manual form rebuilding required.
Who it's for: Registered Managers, Service Managers, and Business Owners who want to digitise legacy paper forms quickly.
Value: Building forms manually in ACP can take hours. Smart Form Builder reduces this to minutes, removing a major barrier to going fully digital.
đĄ Tip: Accepted file types are .docx, .xlsx, .pdf, .png, .jpg, .jpeg, and .heic. Files must be under 5 MB.

Quick Form Submit
Submit a form against any service user's case directly from Evo - without opening ACP. A simple four-step wizard guides you through selecting the service user, case section, form template, and filling in the form.
What it does: Replaces a multi-screen workflow in ACP with a guided wizard that can be completed in under a minute. The submitted form appears immediately in ACP under the correct case section.
Who it's for: Managers and Coordinators who need to quickly log an observation, record a note, or complete an assessment while already working in Evo.
Value: Removes the need to navigate into ACP just to submit a form - saving time and reducing context-switching during busy shifts.
â ď¸ Important: Form submission via Quick Form Submit is pending a platform update. The wizard is fully functional for browsing and completing forms; submission will be enabled once the update is deployed.
Short video walkthrough:
Amend Visit
Review and amend visit actuals for your service users without leaving Access Evo.
What it does: Surfaces visit records where actuals need to be corrected or recorded, with the ability to update details directly or deep-link into the relevant ACP record. This app updates visit actuals - not the planned visit times held in Access Care Rostering (ACR). It is designed for situations where a care worker is unable to record their actuals via the mobile app, allowing a manager or coordinator to do this on their behalf.
Who it's for: Registered Managers and Care Coordinators managing visit accuracy and compliance on behalf of care workers who cannot complete actuals via mobile device.
Value: Keeps visit actuals accurate without requiring repeated navigation into ACP â and provides a clear fallback when mobile recording isn't possible.
Short video walkthrough:
Care Plan Changes
See which service users have had care plan changes, what changed, who changed it, and when - all in one feed inside Navigator.
What it does: Shows a filterable list of care plan changes by date range and branch. Expanding any card reveals the specific field changes (previous value vs new value). Read-only display is fully live.
Who it's for: Registered Managers, Care Coordinators, and Senior Carers who need to stay on top of changes to care instructions before visits.
Value: Reduces the risk of staff acting on outdated care plans. What previously took around 30 minutes of manual checking across individual records is visible in seconds.
â ď¸ Important: The inline Update Field action is pending a platform update. Read-only display of changes is fully live.
Short video walkthrough:
Activity Manager
Monitor care activities across your service users - see what has been completed, what is outstanding, and what has been missed, all from one place in Navigator.
What it does: Displays activity status across your caseload with filtering by branch, date range, and service user. Surfaces missed and incomplete activities so managers can act quickly.
Who it's for: Registered Managers and Care Coordinators responsible for activity completion and compliance.
Value: Gives managers an at-a-glance view of activity delivery without opening individual records in ACP.
Short video walkthrough:
Key Dates
See all upcoming and overdue key dates across your service users - review dates, assessment due dates, contract end dates, and more - directly in Navigator.
What it does: Shows filterable key date cards organised into tabs: Overdue, Due Today, Tomorrow, Next 7 Days, and Next 28 Days. Each tab shows a count badge so you can see at a glance where attention is needed. Clicking any card opens the service user's case in ACP.
Who it's for: Registered Managers, Care Coordinators, and Nurses/Senior Carers who need to track compliance deadlines across their caseload.
Value: Key dates are one of the most common reasons managers log into ACP. This app removes that step - overdue and upcoming dates surface automatically, reducing the risk of missed reviews or lapsed assessments.
Short video walkthrough:
Pending Schedule Updates
See which care workers have rota or schedule changes that have not yet synced to their mobile device - and contact them directly without leaving Navigator.
What it does: Lists staff with unsynced schedule changes, showing what changed, when, and how many service users are affected. Time-window tabs (Next Hour, Today, Tomorrow, Next 7 Days) help you prioritise. A Contact button lets you reach the staff member directly.
Who it's for: Registered Managers, Care Coordinators, and Branch Administrators who need to ensure care workers have up-to-date rotas before their shifts.
Value: Prevents missed or incorrect visits caused by outdated mobile schedules - without managers having to manually track down which carers have not received their updated rota.
Short video walkthrough:
Live View
A real-time monitoring dashboard showing all visits, activities, and alerts across your branches - auto-refreshing every 5 minutes, without you needing to do anything.
What it does: Displays a colour-coded timeline of today's visits (Overdue = red, Scheduled = amber, Complete = green) alongside an alerts panel showing open and closed ACP alerts. Branch and visit status filters let you narrow the view. A current-time marker shows where the day stands.
Who it's for: Registered Managers monitoring live visit delivery throughout the day; Branch Administrators tracking open alerts; on-call staff checking visit status out of hours.
Value: Replaces around 20 minutes of repeated manual checking in ACP with a passive, always-on operational view. Missed visits and open alerts surface in real time.
Short video walkthrough:
Activities Overview
A three-tab performance dashboard showing how well care activities are being closed across your branches and service users.
What it does: The Overview tab shows your overall activity closure rate with automated insights (for example, rate changes vs yesterday). The By Branch tab ranks branches by closure rate and highlights those below threshold. The By Service User tab lets you drill down to individual closure rates. Date range tabs cover Today, Yesterday, Past 7 Days, and Past 28 Days.
Who it's for: Registered Managers, Care Coordinators, and Business Owners/Directors who need to monitor activity completion performance and spot operational trends.
Value: Reduces around 25 minutes of manual report-checking to approximately 10 seconds. Managers who can see closure rate trends daily are better placed to identify systemic issues before they become inspection findings.
