Access Care Planning Evo Connector Overview

Connect Access Care Planning to other systems in your organisation. The Evo Connector gives you access to care data, visits, shifts, forms, medications, users, and more - so you can integrate, automate, and share information across your services.

The Access Care Planning Evo Connector is available as part of the Evo Platform. It gives external systems and tools a way to read and write data directly in ACP - covering everything from visits and care activities to users, forms, and medications.

What you can do

The connector is organised into capability areas - each one covers a specific part of ACP. Here's a plain-English summary of what each area lets you do.


Activities

Read, create, and update care activities. You can also update eMAR (Electronic Medication Administration Record) activities and get a count of activities across your service.


Alerts

Create and manage alerts within ACP - list all alerts, view a specific alert, create new ones, or update existing ones.


Automation rules

Read automation rule action instances linked to a specific case. Useful for checking what automated actions have been triggered.


Branches

Manage the branch records in your organisation - view, create, and update branch information.


Cases

Read and manage case records in full. You can also access the sections, fields, and submitted forms within a case, and export a case as a PDF.


Case documents

Upload and download documents stored within case folders. You can list documents in a folder, upload new ones, or download existing file content.


Case folders

Create and navigate folder structures within a case - create folders and browse their contents.


Comments

Add and retrieve comments, including comments linked to specific care activities.


Customers

Manage customer records and access customer-specific data. This includes eMAR data, activity definitions, workplan links, a live view of customers, and search functionality.


Dashboard

Read and update dashboard configuration, and retrieve summary data for visits and submitted forms.


Devices

Register and manage devices used in your service. You can also retrieve which users are associated with a device, and get a device overview.


Forms

Create and manage forms, retrieve version history, and access the content of specific form versions.


Handovers

Create, update, and list handover records. You can also get a handover overview across your service.


Medication levels

Create and manage medication level records - list all, view individual records, create new ones, or update existing ones.


Medicines

Create and manage medicine records - list, view, create, replace, or update medicine information.


Reports

Create and retrieve reports, and execute them to pull back results.


Services

Read service records and manage service preferences, including viewing sections within a service.


Shifts

Create, manage, and delete shift records. You can also get a shift overview across your service.


Submitted forms

Read and manage submitted form records, including their individual elements and metadata.


Task types

Manage task types and retrieve the forms linked to them.


Users

Create users with a specified role, update user details, and retrieve the groups and devices associated with a user.


User groups

Manage user groups and their membership - list groups, create new ones, and add or remove users.


Visits

Read and manage visit records, including submitted forms and activities linked to a visit. You can also get a live view of visits across your service.


Workplans

Create and manage workplans for customers - view, create, update, replace, or delete workplans.
